Claim Against the City of Portland

File a claim against the City of Portland for property damage or personal injury caused by City operations, employees, or infrastructure. Claims must be filed within 180 days of the incident under ORS 30.275. This form covers both Auto Liability and General Liability claims. Claims are processed by Risk Management.
